Appointment with a palliative care nurse
The palliative care nurse’s appointment is a service that is intended for patients with an oncological disease and their loved ones. A physician’s referral is required.
The service includes the following components:
- Advising patients and their loved ones regarding suitable technical aid, home nursing service, supportive care, hospice, social and support services
- Advising patients and their loved ones regarding the symptoms caused by the disease and/or treatment, and the side-effects of treatment
- Coordinating palliative care, involving different health care specialists and support specialists, referring to services
- Supporting the patient and their loved ones, resolving current issues regarding health
